Dean wakes up with the memory of agony… and finds himself in a casket. He breaks his way through the lid and finds himself in a field with a single wooden cross to mark his grave. He looks around and finds himself in the center of a clearing with the trees blasted away all around.
Dean starts walking down the road to a closed gas station. He breaks in and finds a newspaper that informs him it’s now September. Despite memories of his stomach being ripped open, there’s no sign of an injury. However, there are claw marks on his right shoulder. He starts to steal food and supplies but suddenly the television comes on by itself. He grabs some salt and tries to seal the building but the windows explode inward.
Dean first tries to call Sam but gets a disconnect, and then tries to call Bobby, who hangs up on him. He tries again and Bobby tells him if he tries again, he’ll kill him. Dean hotwires a car and goes to see Bobbi, who grabs a knife and tries to kill him. They fight and Dean tries to get through to him. Finally he proves he’s human by cutting himself with a silver knife. Bobby explains it’s been four months and Sam went off on his own. Sam refused to salt and burn Dean’s body, insisting he’d need it when he got back. Dean figures that Sam did some kind of ritual to bring him back, or made a deal.
Dean tracks down Sam through his cell phone account to Pontiac, IL, near where Dean climbed out of his grave. Dean and Bobby go to the room and find Sam with a woman, Kristy. Sam is astonished to see Dean… and then attacks him. Bobby manages to calm Sam down and the brothers embrace. Kristy leaves and Dean asks how much it cost Sam to bring him back. Sam denies making any kind of deal and explains that he’s been hunting down Lilith to get revenge, and he followed two demons into town. Bobby wonders if Dean feels any different and they decide they need help. Bobby knows a psychic who can help and makes a call. He leaves and Sam gives Dean his necklace back. Dean goes into the bathroom and looks into the mirror, where he has vague flashbacks of what happened to him in Hell.
Once Bobby contacts his psychic, they hit the road with Dean at the wheel. En route, Sam explains that Lilith couldn’t kill him and Ruby is either dead or in Hell. He hasn’t been using his ESP abilities and Dean wonders what other weird abilities he might have. Sam insists that he followed what he thought was Dean’s dying wish not to use his abilities. The next morning, they meet with the woman, Pamela Barnes, who hasn’t been able to locate who might have let Dean out of Hell. She sets up a séance and touches Dean’s wounds to get a link to whatever inflicted them. The table starts to shake and she makes contact with Castiel, who warns her to turn back rather than look upon him. She ignores it and tries to conjure the entity. The candles flare up and Pamela’s eyes bleed and glow white. She collapses to the floor and they call 911, and they discover that her eyes have been burned out.
Later, the brothers grab a meal at a diner and check with Bobby, who informs them that Pamela is stable. Dean wants to track down Castiel, but they’re interrupted when all of the customers approach them and reveal they’re demons. The woman in charge knows about Dean and wants to know how he got out of Hell. Dean doesn’t know and doesn’t believe the demon plans to kill them. He figures they’re just as spooked as he is about it because they want Dean out. She threatens to kill him… and Dean calls her bluff, punching her repeatedly and then walking out. Sam suggests they kill the demons but Dean figures they need to focus on whatever ripped him out of Hell.
Later at Sam’s hotel room, Dean dozes off and Sam slips out. Once he’s gone, the TV in the room switches on by itself, waking Dean up. He draws his shotgun but a high-pitched whining noise causes him to collapse to his knees in pain. The windows blow out and he just avoids the overheard mirror above falling on him. Bobby runs into the room and they hit the road, calling Sam. Sam claims to have grabbed a burger and begs off until later, and Dean accepts his story. He thinks Sam will try to stop them so he didn’t tell him anything.
Meanwhile, Sam is outside the diner from earlier. He goes in and finds a man lying dead on the floor, his eyes burned out. The female demon attacks him and Sam manages to fend her off long enough to discover her eyes are burned out as well. She says they’re all dead and it’s “the end.” She tells him to go to Hell, and Sam raises his hand and forces the demonic essence out of her body and back to Hell. His girlfriend from earlier, Kristy, comes in and knows about Sam’s ability. He calls her Ruby and she admits that whatever they’re up against is a cosmic force beyond any demon.
Dean and Bobby set up at an abandoned building and cover the walls with runes and sigils. Bobby begins the ritual to summon their enemy.
Ruby wonders if Sam is going to tell Dean what they’re doing and how he’s using his psychic abilities. Sam doesn’t think it’s a good idea and insists that they should continue what they’re doing.
Dean and Bobby wait, and the building starts to shake. The lights explode and a man in a trench coat enters the main room. They open fire and he ignores their bullets. The man identifies himself as the one who released Dean from Hell. Dean stabs him with Ruby’s knife… which does nothing. The man renders Bobby unconscious with a touch and says he and Dean need to talk alone. He identifies himself as Castiel: an angel of the Lord. Dean doesn’t believe it and Castiel reveals his wings. Dean still doesn’t believe him, noting that he burned out Pamela’s eyes. Castiel explains that his voice can shatter glass and his true visage can burn out eyes. He thought Dean could handle it but he was wrong. Now he has possessed the vessel of a devout man who prayed to be chosen. He explains that he released Dean because God commanded it, and they have work to do.
