Sam has a horrifying vision of Dean killing a young, demon-possessed man. The vision sends the brothers to Oregon to find answers to Sam's premonition. In Oregon they come across a virus-stricken town where the virus that has infected the town is driving a person to kill. Sam becomes infected by the virus and begins to prepare for his deadly fate, but when the virus doesn't take him as it did the others, Dean decides to tell Sam the truth about what John told him before he died.
