At the abandoned convent in Ilchester, Sam and Dean brace themselves as the Devil begins to emerge from Hell. They run for the doors only to see them swing closed and lock. Unable to escape, they turn around…
… and find themselves on a plane over Ilchester. As they watch, a huge glowing column of light arises from the convent and shoots into the night sky, narrowly missing the plane.
After landing safely, the brothers drive for Chuck Shurley's house and listen to radio newscasts about a sudden plague of disasters and calamities striking across the world. Sam tries to talk about what happened but Dean says not to bother. They try to figure out how they got on the plane, without success, and figure the next step is finding Castiel. They arrive to discover the place has been wrecked and blood is scattered all over the place. Chuck briefly attacks them before realizing who they are, and then explains that the archangel destroyed Castiel when it arrived. Zachariah and his angel assistants arrive and he explains that it's time for Dean to come with them. He explains that they all want the Devil destroyed so Dean should help them. Dean doesn't buy it and Zachariah notices his hand is bleeding. Dean finishing drawing a banishing symbol, temporarily sending Zachariah and the others back to Heaven.
Sam and Dean check into a hotel and Sam makes hex bags to keep the angels and demons from finding them. When Dean wonders how he did it, Sam admits that he learned from Ruby. Dean wonders if his brother is still drinking demon blood and Sam admits that whatever put them on the plane seems to have cleared the addiction from his system. Sam tries to apologize again but Dean abruptly cuts him off. Instead they go to work trying to track down Lucifer.
In Pike Creek, Delaware, a man, Nick approaches his house. As he goes inside, the wind rises and smashes through the trees. He goes to bed but wakes up covered in blood. However, when he turns on the light the blood vanishes. A woman, Sarah, appears in bed with him and says he's been chosen. He blinks and she disappears.
Becky Rosen, a fan of the Supernatural books by Chuck, is surprised when Chuck contacts her via the Internet. He explains that someone is watching him and he needs her to take a message to Sam and Dean, who are real people. She goes to the hotel and is thrilled to finally be able to touch Sam in the flesh. She admits that Dean isn't quite how she pictured him. She tells them that Chuck sent a message of his vision that the Michael Sword is on Earth and the angels have lost it. Now it's on a castle atop 42 dogs. The Winchesters have no idea what that means.
Stumped, the brothers call in Bobby. He arrives at the hotel and explains that the Michael Sword was the weapon that the Archangel Michael used to send Lucifer back to Hell in the first battle between the forces of light and dark. As they start to do the research to figure out Chuck's vision, Sam apologizes and explains that he was the one that killed Lilith, the final seal that released Lucifer. Bobby snaps at him, calling him arrogant and stupid and saying some things can't be forgiven. He tells Sam that once they survive this, he'll never talk to him again. Shocked, Sam leaves to do some research at a nearby church. Bobby and Dean go through the books and Bobby wonders if John Winchester was right and Sam had to be destroyed. He suggests that they should have tried harder to either kill Sam or save him. Dean finally realizes that Chuck's vision refers to Castle Storage at 42 Rover Hill, a storage locker that John used.
As soon as Dean explains, Bobby's eyes turn black. He attacks Dean and two demons burst in. One of them is the Meg demon, wearing a new body. She's happy to kill Dean for trying to oppose Lucifer, and tells the demon possessing Bobby to kill Dean with the demon-killing knife while what's left of Bobby is forced to watch himself kill. Dean tries to get through to Bobby, who throws off the possession long enough to stab himself with the demon-killing knife. Sam comes back and attacks Meg, but finds himself helpless without his powers. Dean manages to kill the other demon and goes after Meg, who leaves her current host body.
In Pine Creek, Nick wanders through the house and finds some toys belonging to his dead child. He hears crying coming from a box and discovers it's coming from a baby monitor. Nick follows the noise to an old nursery but the only thing there is an empty crib. It begins to fill with blood and Nick drops to his knees, crying.
Sam and Dean take Bobby to the hospital and Sam wants to stay, but Dean warns him they need to get to the storage locker and recover the Michael Sword before the demons do. They arrive and discover dead demons on the floor, which is covered with a devil's trap. Zachariah and his two angels are there, and says that Dean and Sam gave the demons the swords. The brothers have no idea what Zachariah is talking about, and he explains that Dean is the sword: he is the chosen vessel for Michael. Zachariah tells Dean to acquiesce and when he refuses, the angel snaps Sam's leg with a casual gesture. Dean realizes that he has to willingly let Michael take his body and refuses. Zachariah offers to heal Bobby, and then inflicts stomach cancer on Dean when he refuses. When he still refuses, Zachariah threatens to remove Sam's lungs. As Sam starts to choke, there's a flash of white light as Castiel kills one of the other angels. After a brief struggle, he kills the other one as well and turns toward Zachariah. Zachariah wonders how he survived and Castiel says it's the same force that got Sam and Dean out of the convent. A frightened Zachariah realizes who he's talking about and Castiel tells him to restore the Winchesters and leave.
Castiel warns Sam and Dean that Lucifer is preparing to take a host body, since all demons and angels must have a vessel. He touches them and engraves their ribs with an Enochian Sigil to prevent any angels from finding them, including Lucifer himself. Sam asks how he survived but Castiel simply teleports away without answering.
Nick wakes up to find what appears to be his wife Sarah standing at the window. She explains that she's really Lucifer and he needs Nick, who has a special destiny. Lucifer warns that Nick will be consumed, mind and body, and it'll be incredibly painful. However, the fallen angel explains that his crime was that he loved God too much. God punished him, and punished Nick by doing nothing when thieves broke in and killed his wife and child. Lucifer offers Nick the chance to get revenge, as long as he willingly agrees to be Lucifer's vessel. When Nick wonders if Lucifer is lying, the fallen angel insists that he can't lie, and doesn't need to. Nick thinks for a moment and then agrees, and a burst of light fills the house.
Dean and Sam go to see Bobby, who disagrees with the doctors who think he'll never walk again. However, Bobby doesn't have any ideas except to save the few they can. Dean figures that there best bet is to find a way to force the angels and demons to leave Earth and fight their battle somewhere else, and he's willing to kill anyone on any side that gets in their way. Bobby wonders how they're going to do that and Dean admits he doesn't have the faintest idea but he won't stop until he succeeds. As the brothers leave, Bobby tells Sam that he heard what the demon told him and insists that he'd never cut Sam out.
Outside, Sam suggests they find the Cold. Dean dismisses the idea, saying that he was just trying to keep Bobby's spirit up. He plans to fight, but he doesn't have any hope that they'll win. When he tells Sam that he should know that if anyone should, Sam finally asks him to say what's on his mind. Dean tells him that he can't trust Sam any more. Sam betrayed him in the worst way possible and there's no way that Dean can overlook it. Sam asks what he can do and Dean says there's nothing he can do. He walks to the car and prepares to leave, while Sam watches him in silence.
