In the Antarctic, Seth is making repairs on a series of wind generators when one of the blades falls off, bounces, and hits him in the leg. He calls for help and a psychiatrist, Dr. Cate Morton, comes to help. She gets him inside and applies emergency first aid, but she starts vomiting and collapses.

House is trying to get cable TV in a patient’s room when Cuddy informs him that he has a new patient, Dr. Morton, and that patients have to pay for cable. They go into differential where they suspect she has a kidney stone, and they have no way to get her out from the South Pole base, or anyone there to treat her. Her medical supplies are limited so Kuttner suggests they use the geological equipment to break apart the kidney stone inside her. While they establish contact by satellite, House goes to Cameron to try to get her to use her influence on the committee to get cable reinstalled for free. She’s less then enthused about the idea and refuses.

House sends his new team to spy on Cameron and get information to convince her to give in to his demands, while they test the sonic device. It doesn’t go well, and Cate disputes she had sex which would be necessary to cause the kidney stone. Taub, Thirteen, and Kuttner inform Cameron they’ve been assigned to spy on her, in the hopes she’ll give in, but she doesn’t.

House argues with Cate when she refuses to take the medication on a theory while others on the staff need it, and she starts to breathe fast, and they determine her lung is collapsing. They walk her through the procedure to reinflate the lung and confirm she doesn’t have a kidney stone. They determine her kidney function is declining and Foreman suspects a tumor. House breaks the news to her and has her do x-rays of her entire body. She does so while House discusses the case with Wilson, who thinks House is hostile toward her because she’s a psychiatrist. They need to do a stain. When Wilson agrees, House notices he’s happy and wearing a new shirt for someone else. House then heads home and contacts Cate, and tells her to take her clothes off to do a scan. She hedges and asks him to show her his place and concludes he has insomnia. Once he shows her, she agrees to strip for the scan except for her socks (because it’s cold) and palpitate for nodes.

While Foreman and Wilson try to determine workable stains, Wilson concludes that Cate is House’s perfect woman: she’s somewhere else and has an incurable disease. Cate begins testing and House spots a node in her navel. She uses ice to numb the skin and injects a needle into the node, but Wilson is impressed that House actually refers to her by name and expresses concern. He suspects House has slept with her, while House notices that Wilson is going out to lunch. When House insists on following him, Wilson runs for it. Cameron then confronts House about his team bugging her and tells him that she’s resigning from the committee… but smirks as she walks away.

House confronts his new team and gives them new instructions to sabotage her by giving them Cameron’s home number on posters advertising dogs with instructions to call in the early morning, so she’ll unresign. Wilson walks Cate through the staining of the node and she reveals she’s been checking up on him. She wonders why he’s House’s best friend and then discovers she doesn’t have cancer. However, Cate feels agonizing pain in her left side, indicating her other kidney is failing.

Back at home, House contacts Cate and discusses her case, suspects auto-immune, and proscribes Prednisone. Cate is reluctant to waste medicine when the other staff members need it. Foreman suggests they send her out in the cold since that will slow down her infection and confirm their diagnosis. Wilson notices that House has stolen his wallet and taken his receipts. House is skeptical about Foreman’s theory, leading Foreman to conclude that House is infatuated with Cate. Foreman notices they don’t have any ANA drugs but they come up a new test: she takes a blood sample and damages them with a paper clip, and the cells will fatten up. She wonders why House has spent so much time with him and he ignores the question.

The next day Cuddy reveals that Cameron made a mistake and is resigning, but House figures she’s lying to get him to give up. They call Cate who reveals the autoimmune test proved negative but she refuses to take the prednisone. Foreman proposes his cold-exposure test and Cate goes along with it. However, Cate collapses as she prepares to go outside.

While Seth tends to Cate, House wonders about Wilson’s girlfriend and who it is. They can come up with a new test: Seth drinks Cate’s urine and goes by the test. Seth obligingly goes along and figures he’s in love with her. House then goes to the new team and condemns them for getting her fired, and Thirteen admits Cameron wasn’t fired. House challenges them to give the right answer and finally Thirteen says she’s done playing House’s games. House says that’s the right answer, and they should stop worrying about being fired and stand up to him.

Seth drinks the urine and it has a watery taster, indicating brain damage. Seth now has to drill a hole to relieve the intercranial pressure, giving them more time to figure it out. Seth balks until House appeals to her love and insists he won’t let Seth hurt her. Seth finally agrees and begins the drill procedure, boring into her temple. As the fluid drains, Cate wakes up indicating their diagnosis is correct. It’s back to differential and Taub suggests blood clots, and Kuttner suggests fatty plaque breaking up. House isn’t interested but Kuttner stands up for his diagnosis and suggests a fat emboli. However, an emboli requires an untreated bone break… and House realizes what part of her body he hasn’t seen.

They go back on camera and have Cate take of her socks. Her toe is broken and bits of bone marrow leaked into her bone stream, causing blockages and accounting for all the symptoms. Seth sets the break and House tells her to thank Seth, and the couple embrace.

Wilson is at dinner placing his order when House shows up, having followed him. House is still trying to figure out who it is. Wilson says that House must really have cared for Cate, but House denies it. Then Wilson’s girlfriend arrives: former associate Amber.
